RESUMO

OBJECTIVE: To evaluate the long-term survival and success rates of implants placed in reconstructed areas using microvascularized or non-microvascularized extraoral bone grafts. MATERIALS AND METHODS: An electronic search was performed in five databases and in gray literature for articles published until June, 2023. The eligibility criteria comprised observational studies (prospective or retrospective) and clinical trials, reporting survival and success rates of implants placed in extraoral bone grafts. A meta-analysis (implant failure) was categorized into subgroups based on the type of bone graft used. The risk of bias within studies was assessed using the Newcastle-Ottawa Scale. RESULTS: Thirty-one studies met the inclusion criteria. The mean follow-up time was 92 months. The summary estimate of survival rate at the implant level were 94.9% (CI: 90.1%-97.4%) for non-vascularized iliac graft, 96.5% (CI: 91.4%-98.6%) for non-vascularized calvaria graft, and 92.3% (CI: 89.1%-94.6%) for vascularized fibula graft. The mean success rate and marginal bone loss (MBL) were 83.2%; 2.25 mm, 92.2%; 0.93 mm, and 87.6%; 1.49 mm, respectively. CONCLUSIONS: Implants placed in areas reconstructed using extraoral autogenous bone graft have high long-term survival rates and low long-term MBLs. The data did not demonstrate clinically relevant differences in the survival, success, or MBL of grafts from different donor areas or with different vascularization. Background: The early detection of impacted teeth is essential for the prevention of future malocclusions and possible lesions. The aim of this study is to assess the prevalence of impacted teeth and associated pathological lesions. Methods: One thousand six hundred and two panoramic radiographs were analyzed for this retrospective study. The prevalence of impacted teeth, the presence of supernumerary teeth and cystic or tumoral lesions associated, and the type of angulation of third molars using Winter's classification were accessed. The descriptive statistics and the Chi-squared test were performed (P < 0.05). Results: One thousand three hundred and ninety-seven panoramic radiographs fulfilled impacted criteria. A prevalence of 541 (38.7%) impacted teeth was observed, with the female gender significantly higher (P < 000.1). Of the total impacted teeth (n = 541), the overall mean impacted of lower third molars was 55.6%. Twenty-nine (5.3%) supernumerary teeth and four lesions were verified. The angulations most observed in the upper (76.6%) and lower (39.2%) third molars were vertical (P < 000.1) and mesioangulated (P < 000.1), respectively. Conclusion: The impacted tooth prevalence was 38.7%, more common in females than in males. The lower third molars presented the highest prevalence of impacted, and the most common angulations were vertical and mesioangulated. Only four associated lesions were observed.

This systematic review (SR) aimed to evaluate the antimicrobial potential of different types of platelet-rich fibrin (PRF) often used in regenerative treatments. An electronic search was performed in four databases and in Gray literature for articles published until January, 2023. The eligibility criteria comprised in vitro studies that evaluated the antimicrobial effect of different types of PRF. For the analysis of the risk of bias within studies, the modified OHAT (Office of Health Assessment and Translation) tool was used. For the evaluation of the results, a qualitative critical analysis was carried out in the synthesis of the results of the primary studies. Sixteen studies published between 2013 and 2021 were included in this SR. The antimicrobial effects of PRF variations (PRF, injectable PRF [I-PRF], PRF with silver nanoparticles [agNP-PRF], and horizontal PRF [H-PRF]), were analyzed against 16 types of bacteria from the oral, periodontal, and endodontic environments. All types of PRF showed significant antimicrobial action, with the antibacterial efficacy being more expressive than the fungal one. The I-PRF, H-PRF, and agNP-PRF subtypes improve antimicrobial activity. According to the OHAT analysis, no study was classified as having a high risk of bias. Evidence suggests that PRF variations have significant antimicrobial activity, with bacterial action being greater than fungal. Evolutions such as I-PRF, H-PRF, and agNP-PRF improve antimicrobial activity. Future studies analyzing the clinical effect of these platelets are fundamental. Implant therapy is considered a predictable, safe, and reliable rehabilitation method for edentulous patients in most clinical scenarios. Thus, there is a growing trend in the indications for implants, which seems attributable not only to their clinical success but also to arguments such as a more "simplified approach" based on convenience or the belief that dental implants are as good as natural teeth. Therefore, the objective of this critical literature review of observational studies was to discuss the evidence concerning the long-term survival rates and treatment outcomes, comparing endodontically or periodontally treated teeth with dental implants. Altogether, the evidence suggests that the decision between keeping a tooth or replacing it with an implant should carefully consider the condition of the tooth (e.g., amount of remaining tooth and degree of attachment loss and mobility), systemic disorders, and patient preference. Although observational studies revealed high success rates and long-term survival of dental implants, failures and complications are common. For this reason, attempts should be made to first save maintainable teeth over the long-term, instead of immediately replacing teeth with dental implants.

BACKGROUND: Zygomatic implants (ZI) have been frequently indicated to rehabilitate patients with extensive atrophies in alternatives to major bone reconstructions. It can be installed inside the maxillary sinus, called instrasinus zygomatic implant (IZI) or outside the maxillary sinus (EZI), depending on the surgery technique. OBJECTIVE: To evaluate the survival and complication rates of ZI in longitudinal studies when compared with conventional implants (CI). METHODS: An electronic search was performed in five databases and in Gray literature for articles published until April, 2022. The eligibility criteria comprised observational cohort studies (prospective or retrospective) and randomized clinical trials (RCTs) with at least 5 years of follow-up, reporting survival rate of ZI versus CI. A meta-analysis was conduct with 18 studies. RESULTS: A total of 5434 implants (2972 ZI and 2462 CI) were analyzed in 1709 patients. The mean survival rate was 96.5% ± 5.02 and 95.8% ± 6.36 for ZI and CI, respectively (mean follow-up time of 78 months). There were observed no statistically significant between ZI and CI in prospective studies (risk ratio [RR] of 1.21; 95% confidence intervals [CIs]: 0.28 to 5.28; chi-squared [Chi2 ] = 11.37; I2  = 56%; degrees of freedom [df] = 5; z-score = 0.25; P = 0.80), retrospective studies IZI (RR of 1.29; 95% CIs: 0.52 to 3.23; Chi2  = 4.07; I2  = 2%; df = 4; z-score = 0.55; P = 0.58) and retrospective studies EZI (RR of 0.72; 95% CIs: 0.31 to 1.66; Chi2  = 1.99; I2  = 0%; df = 3; z-score = 0.78; P = 0.44). The biological complications most related to ZI was sinusitis, followed by infection and oroantral communication. CONCLUSION: ZI have a high long-term survival rate (96.5% with a mean of 91.5 months of follow-up), showing no significant difference when compared with CI. The most prevalent biological complication is sinusitis, being most commonly to the IZI technique. This systematic review (SR) was registered in INPLASY under number INPLASY202280025.

This systematic review aimed to review the literature on the coronectomy technique and evaluate the incidence of success and complications as a surgical approach for inferior third molars. Online databases were searched for data on the frequency of inferior alveolar nerve damage, lingual nerve damage, root migration, pain, infection, dry socket, and extraction of the remaining root, and data on the necessity of reintervention were also extracted. Randomized clinical trials, controlled clinical trials, prospective cohort studies, and prospective and retrospective studies with or without the control group were intercepted. This systematic review was registered in PROSPERO (CDR 42020135485). Sixteen papers analyzed 2176 coronectomies in total, and only five of them were judged as appropriate according to methodological quality assessment. The incidence of inferior alveolar nerve injury was documented in 0.59% of the procedures, lingual nerve injury in 0.22%, infection 3.95%, dry socket 1.12%, extraction of the root 5.28%, and reintervention 1.13%. The pain was the most reported, in 22.04% of the population. This study provides an overview of the clinical success and complications of coronectomy, and their prevalence. A coronectomy may be considered a low-risk procedure and an option for treatment to avoid potential damage to nervous structures. However, patients should still undergo a full screening and evaluation of postsurgical procedures.

BACKGROUND: Although several therapies are proposed for medication-related osteonecrosis of the jaw (MRONJ), to date there is no consensus or treatment protocol for MRONJ. This overview aims to summarize the available evidence on the management and outcomes of MRONJ. METHODS: An electronic search without language or date restrictions occurred in five databases and gray literature for articles published until March 2020. This overview was performed according to the PICO format. The eligibility criteria comprised systematic reviews (RS) with or without meta-analyses that analyzed treatments outcomes for MRONJ. The methodological quality of each SR was evaluated using AMSTAR 2. RESULTS: Fifteen studies published between 2014 and 2020 were included in this overview. The summary of the evidence showed that conservative treatment and low level laser therapy can yield favorable results for early stages of MRONJ whereas surgical treatment seems to be better for advanced stages. However, qualitative assessment of the SRs highlighted a global low level of quality. CONCLUSIONS: There is weak evidence to support treatment protocols for MRONJ. Because of the low quality of the SRs, highlighted by this overview, no insightful therapeutic recommendations can be issued for any intervention for MRONJ.

Introdução: Os casos de assimetria facial são um desafio para tratamento no âmbito da cirurgia Bucomaxilofacial, devido a alteração craniofaciais nos três planos do espaço (Pitch,Yaw e Roll). Estes termos são utilizados para o planejamento da cirurgia ortognática e são essenciais para o planejamento virtual, A utilização da tecnologia neste sentido vem sendo cada vez mais empregada devido a sua previsibilidade de resultado tanto nos casos convencionais como nos mais desafiadores como os das assimetrias faciais. Este artigo visa relatar o planejamento virtual para a correção de um caso de assimetria através da cirurgia ortognática. Relato de caso: Paciente pediátrico leucoderma, do sexo masculino com 5 anos, possui limitação na abertura de boca, desvio da mandíbula para o lado direito com tempo de evolução de 5 meses. O procedimento consistiu na remoção da massa fibrótica através do acesso de Al Kayat. Após o procedimento cirúrgico o paciente foi submetido a uma série de sessões de fisioterapia agressiva. Considerações finais: A associação da remoção da massa fibrótica ou do bloco anquilótico e a fisioterapia intensa no pósoperatório propiciaram um resultado satisfatório, devolvendo a função mandibular do paciente... Introduction: The temporomandibular ankylosis consists in mandibular movements limitation and is characterized as complete fusion of mandibular condyle to articular fossa or as a formation of a fibrotic mass in the region. Case Report: Pediatric male patient, white, 5 year-old, had a mouth opening limitation, mandibular deviation to the right side with 5 mouths of evolution. The procedure consisted in the removal of the fibrotic mass through the Al-Kayat approach. After the surgical procedure the patient was submitted to a series of aggressive physiotherapy sessions. Final considerations: The association of the removal of the fibrotic mass or the ankylotic bloc to intense physiotherapy in the post operatory provides a satisfactory result, restoring patient´s mandibular function... Introdução: A síndrome da apneia obstrutiva do sono (SAOS) é considerada um problema de saúde pública associada a diversas comorbidades que afetam a qualidade e a expectativa de vida. Atualmente, o uso de dispositivos que controlam a pressão do ar durante o sono é considerado uma das melhores terapias para diminuir os sintomas da apneia, entretanto, apresentam difícil adesão dos pacientes. Assim, o tratamento cirúrgico para este distúrbio, cuja cirurgia ortognática se destaca, apresenta alta previsibilidade e bons resultados. Relato de caso: O presente trabalho tem como objetivo relatar o caso clínico de uma paciente do sexo feminino de 40 anos, portadora da SAOS severa e com alto índice de apneia-hipopneia (IAH). Ao exame físico foi observada respiração bucal, deformidade dentofacial do tipo classe II, plano oclusal acentuado e disfunção bilateral da articulação temporomandibular. O tratamento utilizado foi avanço maxilomandibular com rotação anti-horária do plano oclusal associado à mentoplastia. O acompanhamento foi realizado por um período de dois anos, no qual observou uma diminuição no IAH após oito meses da cirurgia e um aumento significativo no volume axial das vias aéreas superiores. Considerações finais: A cirurgia ortognática proposta para este caso demonstrou ser o tratamento adequado, apresentando ótimos resultados e eficácia a longo prazo... (AU)


Introduction: Obstructive sleep apnea syndrome (OSAS) is considered a public health problem associated with several comorbidities that affect quality and life expectancy. Currently, the use of devices that control air pressure during sleep is considered to be one of the best therapies to reduce apnea symptoms, however, it is difficult for patients to adhere. Thus, surgical treatment for this disorder, whose orthognathic surgery stands out, has high predictability and good results. Case report: The present study aims to report the clinical case of a 40-year-old female patient with severe OSAS and a high rate of apnea-hypopnea (AHI). On physical examination, mouth breathing, class II type dental-facial deformity, accentuated occlusal plane and bilateral temporomandibular joint dysfunction were observed. The treatment used was maxillomandibular advancement with counterclockwise rotation of the occlusal plane associated with chin repair. Follow-up was carried out for a period of two years, in which were observed a decrease in AHI eight months after surgery and a significant increase in the axial volume of the upper airways. Final considerations: The orthognathic surgery proposed for this case proved to be the appropriate treatment, presenting excellent results and long-term efficacy... (AU)

Introduction: The facial asymmetry cases are a challenge for the oral maxillofacial treatment due craniomaxillofacial alterations in the three spatial plans (Pitch, Yaw e Roll). These terms are used for orthognathic surgical plan and are essentials for the virtual planning. The application of this technology in this situation has been increasingly employed due to its predictability of outcome in both conventional and challenging cases such as facial asymmetries. Case report: Male, white patient, 30 years-old had a main complaining of facial asymmetry. The procedure consisted in virtual planning trough the orthognathic surgery for facial asymmetry correction, providing higher predictability and stability to the treatment. Introdução:O tratamento da assimetria facial causada pela hiperplasia hemimandibularvemsendodebatidohádécadase, ainda assim,existem algumas controvérsias quanto aomelhor manejo dessacondição. Relato de caso:Revisão de literatura acerca das abordagens cirúrgicas descritas, e, apresentação de caso clínico de uma pacientecomassimetriafacial. Foram realizadas a cirurgiadepredictibil idadenoarticuladorsemi-ajustáveleprototipagem dos maxilares feita a partir de tomografia computadorizada. A paciente foi submetida a cirurgia ortognática com nivelamento da basilar mandibular e condilectomia no mesmo tempo cirúrgico. Introduction: The treatment of facial asymmetry caused by hemimandibular hyperplasia has been debated for decades and, even so, there are some controversies regarding the best management of this condition. Case report: Literature review about the surgical approaches described, and presentation of a clinical case of a patient with facial asymmetry. Predictability surgery was performed on the semi-adjustable articulator and prototyping of the jaws using computed tomography. The patient underwent orthognathic surgery with leveling of the mandibular basilar and condylectomy at the same surgical time. Final considerations: After 48 months of follow-up, the patient has no recurrence and continues with stable occlusion. There is also no pain in the temporomandibular joints. BACKGROUND: Although widely used, the potential for immunological sensitization of allogeneic bone grafts is still controversial in the literature. This systematic review aimed to evaluate the immune response of allogeneic bone graft procedures in humans. METHODS: An electronic search without language or date restrictions occurred in four databases for articles published until July 2019. Eligibility criteria included prospective cohort studies, controlled clinical trials, and randomized clinical trials that evaluated the potential of allogeneic bone grafts in sensitized immune system in humans through immunological or biological analysis. The protocol number of this SR registered in PROSPERO is CDR42019135491. RESULTS: The search and selection process yielded eight prospective cohort studies published between 1984 and 2019. The percentage of patients' sensitization analyzed by human leukocyte antigens ranged from 21% to 67%, with a cumulative mean of 48 ± 17.3%. Bone soluble protein analysis demonstrates the sensitization of 55% of patients after allograft bone grafts. Also, a high level of circulating cell-free DNA took place in the immediate postoperative period. CONCLUSION: Although studies report a low incidence of local complications, an average of 48% of the patients remain with the immune system sensitized after receiving allogeneic bone grafts. Future studies should better address the local and systemic consequences of such sensitization.

A coronectomia, também denominada de odontectomia parcial intencional ou sepultamento intencional, é uma técnica cirúrgica, que visa à remoção da coroa de um elemento dentário incluso, que se apresente próximo a alguma estrutura nobre, evitando, com isso, complicações e/ou riscos desnecessários durante ou mesmo, após a realização do procedimento cirúrgico, como lesão ao nervo alveolar inferior e/ou riscos de uma fratura mandibular. O cirurgião precisa seguir corretamente as indicações e ter um rigoroso planejamento cirúrgico diretamente relacionado a taxa de sucesso. O objetivo deste trabalho é relatar um caso clínico do paciente F.F.C., sexo masculino, 28 anos de idade, leucoderma, submetido à técnica cirúrgica de odontectomia parcial intencional no elemento dentário 37, posicionado abaixo das linhas amelocementários dos elementos 38 e 36, ocasionando a reabsorção radicular da raiz distal deste último. O procedimento foi realizado sem intercorrências no transoperatório. No controle pós­operatório de 24 meses, não houve presença de infecções e /ou exposição do coto radicular do elemento dentário na cavidade bucal, além da ausência de parestesia ou fratura mandibular no pós-operatório... The coronectomy, also called intentional partial odontectomy or intentional burial, is a surgical technique that aims to remove the crown from an included dental element, which is close to any noble structure. Therefore, avoiding unnecessary complications and / or risks during or even after performing the surgical procedure, such as injury to the inferior alveolar nerve or risks of a mandibular fracture. The surgeon needs to follow the indications correctly and have a rigorous surgical planning, directly related to the success rate. The objective of this study is to report a clinical case of a FFC patient, male, 28 years old, leucoderm, submitted to partial denture surgery in the dental element 37, positioned below the amelocemental line of elements 38 and 36, causing a root resorption of the distal root of the latter. The procedure was performed without complications in the trans - operative period. In the postoperative control of 24 months, infections and / or exposure of the dental element in the buccal cavity were not detected, besides not presenting paraesthesia and / or mandibular fracture in the postoperative period... The objective of this study was to evaluate the early osseointegration of two different implants surfaces, a sandblasted and acid-etched surface (TN) compared with same geometry and surface roughness modified to be hydrophilic/wettable by conditioning in an isotonic solution of 0.9% sodium-chloride (TA) through histological and histomorphometric analysis after sheep tibia implantation. Forty dental implants, divided in two groups (TN and TA) were placed in the left tibia of twenty healthy, skeletally mature Santa Ines sheep (n=5/experimental period). After 7, 14, 21 and 28 days post-implantation, the samples were removed and the sheep were kept alive. Analysis of resin sections (30 µm) allowed the quantification of bone area (BA) and bone-to-implant contact (BIC). TA group presented nearly 50% increase in BA at 14 days (p<0.001, ANOVA - Tukey's post test) compared with 7 days. The TA presented higher values than the TN for BA and BIC at 14, 21, and 28 days after placement, stabilizing bone healing. TA hydrophilic surface promoted early osseointegration at 14 and 21 days compared to TN, accelerating bone healing period post-implant placement in sheep tibia.

Autogenous bone grafts are the gold standard for reconstruction of atrophic jaws, pseudoarthroses, alveolar clefts, orthognathic surgery, mandibular discontinuity, and augmentation of sinus maxillary. Bone graft can be harvested from iliac bone, calvarium, tibial bone, rib, and intraoral bone. Proximal tibia is a common donor site with few reported problems compared with other sites. The aim of this study was to evaluate the use of proximal tibia as a donor area for maxillofacial reconstructions, focusing on quantifying the volume of cancellous graft harvested by a lateral approach and to assess the complications of this technique. In a retrospective study, we collected data from 31 patients, 18 women and 13 men (mean age: 36 years, range: 19-64), who were referred to the Department of Oral and Maxillofacial Surgery at the Servidores do Estado Federal Hospital. Patients were treated for sequelae of orthognathic surgery, jaw fracture, nonunion, malunion, pathology, and augmentation of bone volume to oral implant. The technique of choice was lateral access of proximal tibia metaphysis for graft removal from Gerdy tubercle under general anesthesia. The mean volume of bone harvested was 13.0 ± 3.7 mL (ranged: 8-23 mL). Only five patients (16%) had minor complications, which included superficial infection, pain, suture dehiscence, and unwanted scar. However, none of these complications decreases the result and resolved completely. We conclude that proximal tibia metaphysis for harvesting cancellous bone graft provides sufficient volume for procedures in oral and maxillofacial surgery with minimal postoperative morbidity.

The aim of this study was to present a new surgical technique used to remove benign mandibular tumors with minimal damage to the inferior alveolar nerve. The pull-through technique was shown using an ameloblastoma surgical resection as an example. This technique consisted in the reconstruction of the lower jaw associating the resection of the lesion with nerve repair at the same surgical time. The resection was performed using the pull-through technique and the inferior alveolar nerve was preserved. After 6 months, the patient presented a recovery of approximately 80% of sensory function. The surgical technique presented should be considered an important method by which to produce a higher functional outcome to remove benign mandibular tumors with minimal damage to the inferior alveolar nerve and allows the maintenance of quality of life for the patient, as the consequences of this type of surgery are minimized.

Resumo O objetivo deste estudo foi avaliar a osseointegração precoce de duas superficies diferentes de implantes, uma superfície tratada com jateamento e ataque ácido (TN) comparada com outra superfície com a mesma geometria e rugosidade e modificada com hidrofilicidade / molhabilidade através do condicionamento em solução isotônica de 0,9% de cloreto de sódio (TA) através de análises histológica e histomorfométrica após a implantação em tíbia de ovelhas. Quarenta implantes dentários, divididos em dois grupos (TN e TA), foram implantados na tíbia esquerda de vinte ovelhas Santa Inês (n=5/período experimental) saudáveis e esqueleticamente maduras. Após 7, 14, 21 e 28 dias pós-implantação, as amostras foram coletadas e as ovelhas foram mantidas vivas. A análise dos cortes em resina (30 μm) permitiu a quantificação da área de osso (BA) e do contato osso-implante (BIC). O grupo TA apresentou aumento de quase 50% na BA aos 14 dias (p <0,001, ANOVA - pós teste de Tukey) em comparação com 7 dias. O grupo TA apresentou valores maiores do que o TN para BA e BIC aos 14, 21 e 28 dias após a implantação, estabilizando a cicatrização óssea. A superfície hidrofílica TA promoveu uma osseointegração precoce aos 14 e 21 dias em relação à TN, acelerando o período de cicatrização óssea após a instalação dos implantes em tíbias de ovelha.

A miíase oral é relativamente comum, principalmente nos países tropicais e subdesenvolvidos e, na maioria das vezes, afeta pacientes acamados, idosos, moradores de rua e pacientes com necessidades especiais, embora possa acometer pacientes saudáveis. Esse tipo de infecção caracteriza-se pela infestação dos tecidos por larvas de moscas varejeiras - C. hominivorax - que, durante uma fase do seu desenvolvimento alimentam-se dos tecidos do hospedeiro, suas substâncias corporais líquidas ou alimento por ele ingerido. As manifestações clínicas podem variar desde quadros benignos e assintomáticos até formas graves que podem evoluir ao óbito. Os sinais e sintomas clínicos mais comumente descritos incluem a presença de mialgia, febre, odor fétido, inflamação dos tecidos circundantes, ulcerações, necrose tecidual e envolvimento ósseo. O tratamento consiste na remoção mecânica das larvas, auxiliada pelo emprego de terapia medicamentosa local e sistêmica. O objetivo deste trabalho é relatar e discutir o caso clínico do paciente C.A., sexo masculino, melanoderma, 40 anos de idade, atendido no Hospital Municipal Salgado Filho / RJ, apresentando miíase iniciada na região de terço inferior de face e disseminada por toda a região maxilofacial, onde houve uma rápida evolução para óbito em um período de treze horas após sua admissão hospitalar em decorrência de sepse... Oral myiasis is relatively common, especially in tropical and underdeveloped countries, where it most often affects bedridden, elderly, homeless, and special needs patients, but can affect healthy patients. This type of infection is characterized by infestation of the tissues by larvae of C. hominivorax - which during a stage of their development feed on the tissues of the host, its liquid body substances or food eaten by it. Clinical manifestations may range from benign and asymptomatic to severe forms that may progress to death. The most commonly reported clinical signs and symptoms include myalgia, fever, fetid odor, inflammation of surrounding tissues, ulcerations, tissue necrosis, and bone involvement. The treatment consists of the mechanical removal of the larvae, aided by the use of local and systemic drug therapy. The objective of this study was to report and discuss the clinical case of the CA patient, male, melanoderma, 40 years of age, attended at Salgado Filho Municipal Hospital, RJ, presenting myiasis started in the lower third of the face region and disseminated throughout the maxillofacial region, where there was a rapid evolution to death within a period of thirteen hours after admission to hospital due to sepsis... A reabsorção condilar progressiva é uma condição de etiologia controversa, caracterizada pela diminuição gradual do volume do côndilo mandibular, podendo o paciente apresentar, ao exame clínico, sinais e sintomas, como instabilidade oclusal e esquelética, oclusão classe II, mordida aberta anterior, dor e retrognatismo mandibular. Dentre os métodos terapêuticos para o seu tratamento, destacam-se a utilização de enxertos e próteses articulares, associados ou não à cirurgia ortognática para a reconstrução do côndilo mandibular. Este trabalho tem como objetivo relatar o caso clínico de uma paciente do sexo feminino, 28 anos de idade, portadora de artrite reumatoide juvenil, com presença de reabsorção condilar progressiva severa, abertura bucal máxima de 20 mm, dispneia noturna, retrusão mandibular e perfil facial convexo. Ela foi tratada no Hospital Federal dos Servidores do Estado por meio de cirurgia ortognática maxilomandibular com rotação anti-horária do plano oclusal e instalação de prótese condilar bilateral customizada TMJ Concepts®. A reconstrução total proporcionou uma perfeita adaptação à estrutura óssea remanescente, com melhora estético-funcional da paciente. Progressive condylar resorption is a controversial etiology condition characterized by the gradual decrease in the condylar volume, and the patient may present in clinical examination signs and symptoms such as occlusal and skeletal instability, occlusion Class II, anterior open bite, pain and retrognathism mandibular. Among the therapeutic methods for their treatment, it highlights the use of grafts and prosthetic joint, with or without orthognathic surgery for the reconstruction of the mandibular condyle. The purpose this study is to report the case of a female patient, 28 years old, holder of juvenile rheumatoid arthritis, with the presence of severe progressive condylar resorption, maximum mouth opening 20 mm, nocturnal dyspnoea, mandibular retraction and convex facial profile. It was treated at the Hospital Federal dos Servidores do Estado through orthognathic surgery maxilomandibular with counterclockwise rotation of the occlusal plane and installation of customized bilateral condylar prosthesis TMJ Concepts®. The total reconstruction provided a perfect adaptation to the remaining bone structure, aesthetic and functional improvement of the patient. O termo mucocele é empregado clinicamente como um termo genérico, para se referir aos fenômenos de retenção e extravasamento de muco, sendo essa lesão um pseudocisto de etiologia traumática, decorrente do rompimento do ducto de uma glândula salivar menor. Clinicamente, esse tipo de lesão pode se apresentar como uma tumefação ou bolha, macia à palpação, de coloração azulada ou, mesmo, normocrômica, dependendo da sua profundidade nos tecidos, assintomática, de superfície lisa e de tamanho variado, sendo o lábio inferior a região mais acometida. Com relação ao seu tratamento, a enucleação é a técnica mais preconizada. Entretanto, algumas técnicas alternativas têm sido propostas na literatura para esse fim, como o uso do laser de dióxido de carbono, marsupialização e a técnica de Shira, sendo esta realizada com a injeção prévia de um hidrocoloide irreversível no interior da lesão, para evitar o extravasamento do conteúdo mucoso no momento da divulsão cirúrgica. O objetivo deste trabalho é mudar o caso clínico da paciente encaminhada à Clínica de Cirurgia Oral menor da Universidade Federal Fluminense, lhe apresentou, ao exame clínico, uma mucocele em lábio inferior do lado direito, com 14 dias de evolução e cerca de 2,0 cm em seu maior diâmetro, tendo sido realizado remoção completa da lesão por meio da técnica de Shira. The mucocele term is used clinically as a generic term to refer to retention phenomenon and mucus extravasation, which is a pseudocyst injury of traumatic etiology, due to the rupture of the duct of a minor salivary gland. Clinically this type of injury may present as a swelling or blister, soft palpation, bluish or even normochromic coloring, depending on their depth in the tissues, asymptomatic smooth surface and varying size, with the lower lip the most affected region. Regarding treatment, enucleation is the recommended technique. However, some alternative techniques have been proposed in the literature for this purpose, such as using carbon dioxide laser, Marsupialization and Shira's technique, which is performed with the previous injection of an irreversible hydrocolloid inside the lesion to avoid extravasation the mucous content at the time of surgical divulsion. The aim of this paper is to present a case of C.R.F. patient, 18-year-old female was referred to minor oral surgery clinic of the Federal Fluminense University, with clinical examination mucoceles in lower right lip 14 days of evolution and about 2.0 cm in its largest diameter, where complete removal of the lesion was performed using the Shira's technique.
